NDIS Support Coordination Services at Designer Life

Level 1 – Support Connection
Registration Group: 0106

For participants who want to build confidence and take more control of their plan.

Designer Life can support you with:
•  Assistance in understanding your NDIS plan and what’s funded
•  Introductions to local service providers and community supports
•  Guidance on how to manage your supports independently
•  Support with setting up service agreements and bookings
•  Answering any questions you may have as they arise during your plan

Level 2 – Coordination of Supports
Registration Group: 0106

For participants with more complex needs or those who may benefit from ongoing coordination and coaching.

Designer Life can support you with:
•  Developing a tailored support strategy based on your individual goals
•  Assist you to connect with a mix of supports to assist you in meeting your goals
•  Monitor your plan budget and adjust supports as needed
•  Help resolve service delivery issues and reduce barriers
•  Build your capacity to manage supports and advocate for your needs
•  Prepare for plan reviews and provide progress reports to the NDIA

Note: We do not offer Level 3 - Specialist Support Coordination.

How We Support Choice, Control, and Compliance

At Designer Life, we are committed to upholding the highest standards of integrity, transparency, and participant choice and control in line with the NDIS guidelines on conflict of interest. We have a strict internal policy that ensures:

  • No Dual Services Delivery by the Same Staff Member: A staff member providing Support Coordination will never deliver another funded service to the same participant. This separation is essential to avoid any real, perceived, or potential conflict of interest, as outlined by the NDIS Code of Conduct.
  • Separate Service Agreements: Support Coordination and any other services offered by our organisation are governed by distinct service agreements. This ensures clarity, transparency, and informed consent for each type of support.
  • Multiple Tailored Recommendations: When referring participants to other services, our Support Coordinators always recommend at least three options (unless less than 3 appropriate options available to the participant, for example due to a remote servicing location) tailored to the participant’s goals and support needs. These recommendations will be documented against suitability for the participants requirements. These options include providers both within and outside our organisation, empowering participants to make independent, informed decisions.
  • Documentation and Disclosure: Any potential conflict of interest is documented and disclosed to all relevant parties. We maintain accurate records and follow the NDIS Practice Standards to ensure compliance and accountability.
    This policy reflects our commitment to participant choice and control.
